Physical Therapist (AL/IL & Home Care) (9528)

Join Summit Home Care and Hospice as a Physical Therapist (PT) to support and treat patients in Assisted Living (AL) and Independent Living (IL) facilities, and in-home visits. You’ll enjoy a consistent caseload, reduced travel time, and the chance to build strong relationships with patients and facility staff.

• Location:
  
  Chillicothe, OH & surrounding areas (including Washington Courthouse & Circleville)
• Schedule:
  
  Full-Time (Monday – Friday, 40 hours/week, flexible scheduling)
• Salary: $80,000 – $110,000 per year
• Travel required:
  
  Yes – mileage reimbursed at $0.48/mile

• Help patients regain mobility and independence
• Deliver physical therapy services per physician’s care plan and state regulations
• Assess patient mobility, strength, and rehabilitation potential
• Design and implement personalized therapy programs
• Educate patients and caregivers on therapeutic techniques and equipment
• Collaborate with interdisciplinary teams to ensure holistic care
• Supervise PTAs and home health aides as needed
• Document progress and participate in discharge planning

• Active Ohio PT license
• Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
• CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Recent TB test

• 2+ years of PT experience
• Home health or community-based care experience
• Strong communication and organizational skills
• Ability to build rapport and lead with empathy
