Overview

Nurse Case Manager job copee, MA.

• Medical Oversight & Coordination:
  Supervise and coordinate healthcare services for residents in Western MA programs. Attend medical appointments and ensure appropriate follow-ups. Advocate for optimal health and well-being for all individuals.

• Work closely with physicians, nurse practitioners, and medical providers.
• Communicate effectively with the Area Director, Nursing RN Supervisor, Health Care Director, and other DDS staff.

• Maintain accurate records, including progress notes, medical logs, and staff communication updates.
• Ensure compliance with MAP standards for medication and treatment orders.

• Educate staff on personal care, positioning, feeding, and supportive device usage.
• Provide ongoing nursing guidance and support to direct care teams.

• Conduct quarterly audits of residential programs.
• Perform other assignments as directed by management.

• Valid MA Nursing License (LPN or RN)
• Valid driver’s license and acceptable driving record
• Strong communication and computer skills
• Physical ability to perform job duties

Salary is determined by relevant experience, education, and skills. LPN Starting Pay: $30-$35/hr rn Starting Pay: $35-$39/hr.
