Setting:  Skilled Nursing Facilities (SNFs)

Position Overview
We are seeking an experienced  Family Nurse Practitioner (FNP)  to provide high-quality primary medical care to  adult and geriatric patients  in Skilled Nursing Facilities. This role is ideal for a provider who is comfortable working independently and has experience in post-acute or long-term care settings.

Key Responsibilities

Provide primary medical care to adult and geriatric patients in SNFs

Perform routine rounds and follow-up visits

Manage chronic conditions and respond to acute changes in patient status

Coordinate care with nursing staff, facility leadership, and interdisciplinary teams

Complete timely, accurate, and compliant medical documentation

Ideal Candidate Profile (Strong Fit)

Certified  Family Nurse Practitioner (FNP)

Experience in one or more of the following:

Skilled Nursing Facilities (SNF)

Post-acute or long-term care

Hospitalist or geriatric care

Comfortable practicing independently

Prior locums or contract experience is a plus

Not a Fit

New graduates  without  SNF or adult medicine experience

Pediatric-only providers (PNP)
