Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in‑home care and case management for older adults, children, and those living with disabilities across many counties. You are a Certified Home Health Aide in California who is interested in working with an interdisciplinary team of health professionals helping individuals that have transitioned from a hospital to home care setting and assisting them to remain in the community. You enjoy helping clients achieve their personal goals of remaining in the community. You want to work with a team that inspires professional growth and share our priority of providing quality case management and home health care to the community.

* This is not a position for Caregivers, or Registry Aides. We are accepting applications from Certified Home Health Aides who have completed training as a Home Health Aide and received the necessary license issued from the State of California.

• Assists patient with personal care activities including bathing, oral hygiene, preparation and feeding of meals, dressing and undressing, ambulation, and other activities as needed
• Performs duties as assigned in an accurate and timely manner
• Notifies the office of any abnormal findings or conditions

• Patient-interaction skills
• Experience with ADLs
• Experience with Hoyer Lifts
• Ability to multi-task and perform duties in a timely and orderly manner

• Completed an approved Home Health Aide training course and received Certification as Home Health Aide from the State of California
• Valid California Driver’s License, and eligible to be insured under our liability policy with a clean DMV report
• Your own vehicle and proof of current auto insurance
• Current CPR certification

PPE is provided to all employees

This is not a remote role and requires you to work out of the Chico office seeing patients in Chico and Butte County and in surrounding counties.

Part‑time work - Ranges from 2–29 hours per week, hours may vary.

You will need your own car and be able to travel to client’s homes within the Chico area and surrounding counties.

Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirements for this role specifically include:

• Sits, stands, bends, lifts, walks, and moves intermittently during working hours
• Able to lift to 50 pounds without assistance
• Able to drive intermittently

You will be reimbursed for the mileage you travel from the office to client’s homes.

You will receive a monthly phone stipend.

Eligible for sick benefits after 90 days.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
