The RoleAs a Care Coordinator, you will be central to the day-to-day running of the Chichester branch. You will support service delivery, maintain continuity of care, and work closely with clinical and operational teams to meet both care quality and business objectives. This role suits someone who thrives in a fast-paced, people-focused environment.

- Clients and families
- Healthcare professionals and social workers
- Case managers and external service providers
- Ensure all client visits are covered daily
- Prepare and maintain rotas at least four weeks in advance
- Coordinate quality visits for Field Care Supervisors and Managers
- Liaise with Branch Nurses on complex care, assessments, training, and competencies
- Deliver excellent customer service to clients and colleagues
- Communicate care plan updates and rota changes clearly
- Allocate care staff based on skills, preferences, and client needs
- Maintain accurate records and ensure compliance with systems and procedures
- Build and maintain strong relationships with clients and frontline teams
- Identify local business development opportunities with the Branch Manager
- Participate in the on-call rota, including 1:4 weekend cover
- Support neighbouring branches when required
- Prepare reports and support day-to-day branch operations

- Previous experience in care coordination or care planning
