Reports to: Program Manager

Mission: We aim to build healthy and peaceful communities.

Service Coordinator:

The Service Coordinator (SC) will act as a convener, advocate and organizer of resources to support youth that have been justice impacted, disengaged from school and or at the highest risk of being the victim or perpetrator of violence. Service Coordinators will carry a caseload of no more than 15 youth at a time. The SC should have adequate experience in working with youth who have experienced trauma.

They should understand the importance of race, cultural background, gender, sexuality and any other characteristics youth may identify with or as. The SC should serve as a problem solver with youth to address their immediate goals and needs without judgment, be encouraging, help build self-esteem, be motivating and to overall support youth in identifying things that keep them productive and safe.

Program Specific

Duties and Responsibilities:

  • Conduct intake assessments with youth & family for case planning, referrals, follow-up, and case closure
  • Connecting with youth through phone calls, home visits, school visits or meeting in neutral locations to engage youth in services
  • Utilize assessment tools to identify youth strengths, needs, and pathways to connect them to services
  • Plans and implements activities that are in alignment with the core components of the program
  • Empowers youth in decision-making
  • Engage families as appropriate and pursue family service connections that support youth well-being.
  • Enter and track data to monitor youth outcomes. Effectively communicates with referral partners of any changes to youth’s circumstances that impact service delivery

Preferred Qualifications

  • Bachelor’s Degree in social work or a related field. High school diploma/equivalent plus three years’ relevant experience
  • Familiarity and experience working with youth, adults and families
  • Committed to the mission of Think Outside Da Block
  • Demonstrates strong interpersonal skills, ability to develop trust, ability to collaborate and work as a team with colleagues & participants
  • Demonstrates effective written and oral communication skills; able to communicate clearly and concisely for various mediums & audiences.
  • Demonstrates excellent organizational skills, able to execute ideas, ability to manage multiple tasks and meet deadlines
  • Demonstrates excellent attention to detail.
  • Demonstrates basic computer skills, including Microsoft Office, Outlook and client electronic data systems
  • Demonstrates respect and acceptance of diverse individuals and communities
  • Served by, working in partnership with or employed by Think Outside Da Block including but not limited to race, gender, ethnicity, social economic status, and LGBTQ populations.
