Community Health Nurse
The Community Health Nurse will provide direct client care, health education, and coordination of care. The role is accountable for coordinating and facilitating clients’ services across the various levels of the care (outpatient, inpatient, specialists, hospital admissions, respite care, etc) for the Front Door Diversion team clients. The nurse will work with clients directly in the community (approximately 70% of the time), engaging them in community settings.

Pay: $82,800 - $95,220 annually
$3,000 Sign-on Bonus!
Work Schedule: Monday – Friday, 8:00 am – 4:30 pm
The Community Health Nurse will provide supervision and consultation for the team and clients on illness, self‑management, and medication monitoring, training, etc. They will collaborate with the treatment team, providers, and others on client plans of care to ensure quality standards are met. The nurse will provide strengths‑based, client‑centered, and trauma‑informed services to clients with severe mental illness and co‑occurring substance use disorders, utilizing a harm reduction approach and motivational interviewing skills.
Responsibilities- Make initial and ongoing assessments and develop treatment plans outlining nursing interventions.
- Address health needs of clients with serious mental illness and substance use disorders, including administering specified medications (e.g., long‑acting injectable antipsychotics), obtaining vital signs, and performing point‑of‑care testing.
- Document in electronic health records per Trilogy’s policies for assessments, injections, and progress notes.
- Collaborate with clinical and non‑clinical staff on client care plans and ensure quality standards are met; evaluate effectiveness, monitor progress, and effect change as needed.
- Attend agency‑wide nursing meetings, supervision meetings with supervisors and the nurse manager.
- Ensure clients and staff receive education related to individual clients’ health needs.
- Perform and document medication reconciliation in EMRs.
- Advocate for clients, collaborate with the team, and link clients to community services (e.g., medical specialists) for complex coordination of care.
- Coordinate client transport to and attendance at medical appointments in the community as necessary.
- Provide individualized motivational treatment to support recovery from mental illness.
- Deliver client‑centered and trauma‑informed services to clients with severe mental illness and co‑occurring substance use disorders.
- Attend daily team meetings to provide updates on client needs.
- Perform other related duties or projects as assigned to support the agency.
- Current licensure as a Registered Professional Nurse with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
- Valid Illinois Driver’s License with daily access to a vehicle; must have $100,000/$300,000 auto liability insurance.
- Experience in a community mental health setting or with psychiatrically disabled or other disabled populations highly preferred.
- Experience in care coordination highly preferred.
