Salary:
The previously determined range for this position was $40.08/hr to $91.37/hr*. Final rate offered will be determined by the collective bargaining agreement, internal salary equity, candidate experience and qualifications, and budget constraints. It is not typical for an individual to be offered a rate at or near the top of the full range for a position. This position requires Illinois residency within 180 days of the hire date.

Position Summary

The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation. Practicing under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase. The job duties vary with the particular setting or assignment.

Transplant Nurse Coordinators can be assigned to kidney, pancreas, liver and/or small bowel programs. Clinical Nurse Consultant II is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Clinical Nurse Consultant II provides patient care and consultation, coordinates the educational development of staff members and acts as a role model in clinical practice.

Duties & Responsibilities
  • Evaluates patients at the appropriate UI Health site of care.
  • Coordinates care for transplant recipients from referral and/or until waitlisted, all potential recipients with living donors and all living donor patients.
  • Ensures that all pre-transplant patient coordination and patient education is documented and is in compliance with UNOS, CMS transplant program guidelines and UI Health policies/procedures.
  • In collaboration with the provider, orders all testing for evaluation as needed and reviews results with the provider in a timely manner following UNOS and CMS guidelines.
  • Develops, coordinates, and communicates the patient s plan of care based on ongoing assessment of the patient and input from the patient s family.
  • Uses clinical expertise to collaborate with multi-disciplinary health care team to improve patient outcomes.
  • Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record.
  • Assesses, engages and obtains clinical information from multi-disciplinary team, patient and/or family to develop an appropriate patient educational plan.
  • Recognizes symptoms, establishes priorities and responds to patients changing needs throughout the transplant care continuum.
  • Ensures that the patient is added to the UNOS waiting list and the required listing letters are sent in a timely manner per UNOS requirements.
  • Coordinates and schedules all living donor transplant surgeries as assigned.
