Be a part of a world-class academic healthcare system, University of Chicago Medicine, as a Patient Logistics RN in the Patient Logistics department!
The Patient Logistics RN assigns patients to beds based on care requirements and for maximizing the use of the hospital's bed resources. In determining priorities and reaching bed assignment decisions, the Patient Logistics RN operates within the bounds of established hospital policy and resource availability guidelines. The Patient Logistics RN will evaluate, process, track, document and coordinate patient transfers into University of Chicago Medicine.
The Patient Logistics RN obtains appropriate medical and demographic information, and coordinates communication between the referring physician and the University of Chicago Medicine. The Patient Logistics RN efficiently engages appropriate departments in the transfer process and monitors and documents all activities associated with the transfer. The Patient Logistics RN is responsible for oversight of the entire transfer episode, from start to finish, and for communication back to the transferring facility.
The incumbent will be accountable for all regulations associated with patient transfer as well as designated performance measure outcomes.
Essential Job Functions
* Assigns patients to beds based on UCH established priorities for bed assignment and other bed assignment indicators to minimize bed down time, maximize the clinical usefulness of each bed day and the meet the convenience of patients.
* Works in concert with the Director to track the status of all Requests for Admission, both scheduled and same day.
* Provides routine feedback to admitting where there are issues of accuracy, completeness or lateness of delivery.
* Tracks the status of beds and patient waiting time in the ERs.
* Participates in discussions when the ERs are considering going on bypass.
* Ensures that timely and appropriate transportation is facilitated.
* Serves as the principal contact for requests to transfer a patient from another hospital or other outside setting to University of Chicago Medicine.
* Evaluates the appropriateness of bed assignments.
* Provides direction to others in achieving the necessary changes.
* Collaborates with all hospital departments and other disciplines to streamline services and effectively monitor the utilization of resources.
* Prepares written reports and oral presentations regarding variances or cost effectiveness
* Addresses and escalates as appropriate issues related to transfer acceptance, bed availability and increased census
* Knowledge of State and National rules and regulations pertaining to healthcare transfer centers and Bed Access, including EMTALA.
* Routinely assesses indicators and monitors concerning bed availability and the maintenance of care standards for assigned area.
* Utilizes indicator information to determine when and what interventions are required. In pursuit of this goal, facilitates the timely discharge of patients.
* Obtains appropriate patient information critical to determining the appropriate University of Chicago Medicine care provider, medical necessity and level of care.
* Facilitates a recorded conference call between the referring physician and the University of Chicago Medicine physician.
Required Qualifications
* Shift changes, on-call duty and other non-standard work cycle.
* Graduate of a NLN accredited school of nursing
* BSN in Nursing Required
* Minimum of 5 years progressive clinical experience, to include acute care cardiac telemetry, critical care/emergency nursing experience, case management, charge nurse/house supervision, or transfer center experience
* Exceptional communication skills, both written and oral.
* Strong interpersonal and organizational skills, leadership, teamwork and collaboration
* The ability to resolve conflict effectively.
* Excellent critical thinking skills and ability to think quickly and multitask effectively.
* Requires the ability to complete job duties autonomously.
* Ability to use a computer and software applications.
