What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managersprovide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours:
- 40 hours per week 8:00 am - 4:30pm
- Monday through Friday
- No Night Shifts (also known as PM and NOC Shifts)
- Limited on-call rotation
- $34- $37 per hour starting wage, depending on experience

Qualifications /
Job Requirements:
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
