, is currently seeking to hire a compassionate and dependable full-time Licensed Practical/Vocational Nurse (LPN) in the Hayward area to join our home health team. What does a Transitions at Home LPN do?

Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours:
Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Limited on-call rotation

• $28- $32 per hour starting wage, depending on experience

Benefits vary by employee status (full-time, part-time, prn)

• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance
