Position: Restorative Nurse (Chicago - South Loop)
Overview
We are seeking a dedicated and compassionate Restorative Nurse to join our team. The ideal candidate will be passionate about improving quality of life for residents through restorative nursing practices. In this role, you will take a leading part in creating and implementing individualized care plans that promote independence, mobility, and overall wellness.

Key Responsibilities

• Partner with the interdisciplinary team to assess residents and develop comprehensive restorative care plans.

• Implement restorative interventions that support residents’ physical, mental, and psychosocial well-being.

• Provide hands-on support and guidance to nursing staff on safe transfer, ambulation, and activities of daily living techniques.

• Monitor resident progress, adjusting care plans as needed to ensure optimal outcomes.

• Accurately document interventions, progress, and changes in condition.

• Perform regular assessments of mobility, range of motion, and functional ability, and share findings with the care team.

• Coordinate with rehabilitation therapists to align restorative nursing activities with therapy goals.

• Educate residents and families on restorative programs and strategies for maintaining independence.

• Maintain a safe, supportive environment that fosters restorative care practices.

• Stay informed on current best practices in restorative nursing and apply them in daily care.

Qualifications

• Active RN or LPN license in the state of practice.

• Prior experience in long-term care or geriatric nursing preferred.

• Strong clinical assessment and critical thinking skills.

• Excellent communication and interpersonal abilities.

• Ability to work independently as well as collaboratively within a team.

• Compassionate, resident-centered approach to care.

• Knowledge of restorative nursing principles and techniques.

• Commitment to ongoing learning and professional growth.
