Salary range: $38.50 – $61.60 per hour. Sign‑on bonus: $4,000.

Responsibilities
  • Serve as a patient advocate in an ambulatory setting and assist patients in navigating across care settings.
  • Address the care‑coordination needs for high and moderate health‑risk patients, including low‑risk or new patients.
  • Review patient records, registries, reports, or other encounter and claims data to identify patients who may require care coordination.
  • Assess, document and address clinical, psychosocial or financial barriers to effective patient care.
  • Consult with multidisciplinary teams to develop, implement and evaluate care plans tailored to patient needs.
  • Provide effective triage care using in‑depth clinical knowledge and skills in the area of expertise.
  • Offer educational support to patients and family members regarding medical processes, treatments, medications and wellness management.
  • Ensure timely continuity of care by proactively outreaching to patients post‑discharge, managing referrals, facilitating transitions, and coordinating community resources.
  • Manage high‑risk patient cases to minimize readmission rates and reduce costs.
  • Develop comprehensive care plans aligned with patient goals and values; inform patients on preventative care practices.
  • Maintain accurate patient health information for provider and care‑team use.
  • Monitor quality of care and satisfaction; participate in continuous quality improvement initiatives.
  • Provide after‑hours on‑call support for urgent care coordination issues.
  • Perform related duties as directed or required.
Qualifications Required
  • Current license as a Registered Nurse – State of Illinois Professional Nurse.
  • Bachelor of Nursing Degree (BSN) with 3–5 years of relevant clinical experience (or a BSN within two years of hire).
  • 3 years of experience (5 years preferred) in chronic conditions, evidence‑based medicine, care coordination, care management, and psychosocial and behavioral factors affecting health outcomes.
  • Willingness to travel (some travel may be required).
Preferred
  • 3 years of experience in Care Management or Care Coordination.
  • Case management certification preferred or willingness to complete upon eligibility.
  • Experience with ambulatory operations and ambulatory nursing care.
  • Experience with process improvement.
Sign‑On Bonus Eligibility

External candidates and candidates who have not worked at NM within the last year are eligible for the $4,000 sign‑on bonus. Internal employees and rehires who left Northwestern Medicine within one year are ineligible (except new graduate internal employees who may qualify depending on the job family).
