Service Coordinator Chicago, IL Alpha Phi Alpha Homes, Inc.
is currently seeking a full‑time Service Coordinator to join our team in Chicago, Illinois! Formed in 1966 by the members of the Alpha Phi Alpha Fraternity, Inc., we are a non‑profit housing, development, and management corporation fund.

- Pay: $20.50 per hour
- Monday to Friday
- 8 AM – 5 PM
- No weekends
- Must have valid Driver’s License
- Bachelor’s degree (preferred) or appropriate work experience
- Appropriate work experience in related field
- Social Work or Professional Service Coordinator designation (preferred)
- Must comply with all AASC (American Association of Service Coordinators) requirements annually
- Ability to build and maintain relationships with diverse backgrounds
- Ability to manage deadlines and progress across the team to ensure projects are delivered on time and on budget
- Demonstrated working knowledge of local services for the elderly and people with disabilities
- Excellent written and verbal communication
- Microsoft Office
- Assist and advises residents and families of the services which may be necessary to age in place
- Promote and facilitate wellness and educational activities for all residents
- Ensure residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change
- Provides general case management (including intake), assessments and referral services to residents needing assistance
- Acts as the clients' advocate, explains the available options to the clients, and keeps records of service
- Attend tenant meetings
- Set up directory of providers for use of staff and residents
- Prepares various reports and government documents
- Establish relationships with local agencies and service providers for the residents
- Must be able to maintain confidentiality and work with elderly and disabled individuals
- Maintain a good relationship with residents, Property Management, and stakeholders
- Maintain timely and quality documentation and data entry

Alpha Phi Alpha Homes, Inc. is a non‑profit housing, development, and management corporation formed in 1966 by the members of the Alpha Phi Alpha Fraternity, Inc. Eta Tau Lambda chapter. Alpha Phi Alpha Fraternity is the oldest predominately black collegiate Greek‑Lettered fraternity. The Eta Tau Lambda is an alumni chapter of Alpha Phi Alpha, located in Akron, Oh, and was formed in 1960 specifically to support their local communities.
