Position Summary
The Substance Use Disorder (SUD) Navigator serves as a key member of the care team, providing direct support to individuals affected by substance use. The Navigator helps patients access appropriate treatment, recovery, and community resources while reducing barriers to care. This role focuses on engagement, education, coordination, and advocacy to promote sustained recovery and improve health outcomes.
Essential Duties and Responsibilities- Conduct screenings and assessments to identify substance use concerns and determine appropriate levels of care.
- Engage patients in a nonjudgmental, trauma-informed manner to build trust and support readiness for change.
- Provide education on substance use disorders, treatment options, harm reduction strategies, and recovery supports.
- Assist clients in navigating healthcare systems, behavioral health services, and social service programs (e.g., housing, employment, insurance).
- Coordinate referrals and facilitate warm hand-offs to treatment providers and community resources.
- Follow up regularly with clients to ensure linkage to care and continued engagement in treatment or recovery programs.
- Collaborate with multidisciplinary teams including physicians, social workers, case managers, and peer support specialists.
- Document all encounters and interventions accurately in the electronic health record (EHR) or case management system.
- Track and report client outcomes, service utilization, and program metrics as required.
- Participate in team meetings, training, and quality improvement initiatives.
Education and Experience:
- High School Diploma
- Minimum of 2 years of experience working in behavioral health, substance-use treatment, care coordination, or case management.
- Knowledge of substance-use disorders, recovery principles, and harm reduction practices.
- Familiarity with local treatment systems, community resources, and public benefits.
- Certified Alcohol and Drug Counselor (CADC) and Certified Peer Recovery Specialist (CPRS), or equivalent preferred.
- Valid driver’s license and reliable transportation (if field-based).
- Strong interpersonal and communication skills.
- Ability to engage individuals from diverse backgrounds with empathy and cultural humility.
- Organized, detail-oriented, and capable of managing multiple priorities.
- Skilled in motivational interviewing and trauma-informed approaches.
- Commitment to recovery-oriented and person-centered care.
- Proficient in documentation and electronic health records.
- Work performed in clinical, community, and/or office settings.
- May involve travel between sites and client homes.
- Occasional evenings or weekends required based on client needs.
- Ability to sit, stand, and walk for extended periods.
- Ability to lift up to 25 lbs occasionally.
