Weekend Liaison & Admission Reviewer

Overview

Aliya Healthcare is seeking a dedicated and compassionate professional to serve as our Weekend Liaison & Central Admission Reviewer. This dual-role position is essential for ensuring high-quality care coordination and seamless communication between hospitals, families, and our skilled nursing facilities. The individual will be responsible for reviewing all weekend referrals for clinical appropriateness and admission planning.

Weekends Only (Saturday & Sunday)

• Hospital Liaison Duties:
  Serve as the primary point of contact between hospital staff, facility staff, and families.
• Communicate clinical updates and discharge planning needs back to the respective nursing facilities.
• Advocate for resident needs and ensure their preferences are respected during hospitalization.
• Coordinate with discharge planners and case managers to ensure a smooth transition back to the facility.
• Maintain detailed records of all hospital visits, communications, and follow-ups.

• Review and assess weekend admissions referrals for clinical appropriateness.
• Coordinate with hospital discharge planners, referring providers, and internal admission teams regarding pending referrals.
• Identify any potential barriers to admission and communicate necessary information to the facility leadership.
