Responsibilities- Offers case management/care coordination support services to individuals in Cook County Jail including assisting with Medication Assisted Recovery, residential treatment, other resources and coordination of cases with various comprehensive team members.
- Works closely with the medical providers and care coordination team (which may include legal team) to provide outreach and engagement services.
- Supports specialized screening, referral and warm-hand-off services to substance use disorder treatment and recovery support services, including Medication Assisted Treatment for individuals with Opioid Use Disorder.
- Conducts informational/educational sessions providing appropriate materials to educate patients on addiction recovery and aftercare options.
- Acts as coordinator for discharge needs of assigned patients.
Monday - Friday (8:00 a.m.
- 4:30 p.m.).

Starting Pay RateUp to $22.00 per hour

ClearanceDCFS clearance required.

Required ExperienceEducation and/or
Experience:
- High School diploma or equivalent is required.
- Bachelor's in Psychology, Social Work, or counseling degree is preferred but not required.
- Substance Use Disorder lived experience required.
