A community health organization in Chicago is seeking a Community Health Worker (CHW) to enhance care for individuals affected by HIV. The CHW will independently promote health services, coordinate patient care, and maintain collaborative relationships within the community. Candidates must have a High School Diploma or GED, a minimum of two years of relevant experience, and a Community Health Worker Certification.
