Job Title: Resident Service Coordinator
Location: Christian Community Health Center, Chicago, IL
Employment Type: Full-Time

Pay Range: $50,000.00 – $50,000.00 per year
Position SummaryThe Resident Service Coordinator will serve as part of the supportive services team and will focus on building a system to support collaborative relationships with residents to impact their health, well‑being, and financial stability goals. The role involves intake of potential residents, coordination of resources, data analysis to drive program decisions, case management and referral coordination, and supporting peaceful community living.
Minimum Qualifications- Bachelor’s degree in Social Work or related field, with a minimum of 3-5 years of recent related experience.
- Associate’s degree with at least five years of recent related experience.
- A valid Driver’s License and vehicle insurance coverage.
- Occasional late or weekend hours may be required. Strong oral and written communication skills, excellent engagement skills, and the ability to work effectively with a diverse group of people from all social and economic backgrounds.
- Basic computer skills.
- Deliver care coordination services (resident assessment, advocacy, planning, facilitation, monitoring, and evaluation of resources to meet residents' specific needs).
- Provide coordination and referral services for physical, emotional, health, substance abuse treatment, employment, and other resources.
- Coordinate and/or lead workshops or classes on life skills, employment readiness, job searches, benefits, entitlement programs, financial literacy, health and wellness.
- Plan and coordinate monthly events.
- Participate in monthly meetings with Property Manager and key staff to discuss trends and improve service delivery.
- Maintain resident service documentation, logs, and notes while ensuring confidentiality.
- Conduct resident satisfaction surveys, compile and present data for recommendations and process improvements.
- Prepare and distribute a monthly calendar of events for residents.
- Prepare and submit monthly service reports to direct supervisor.
- Participate in supervision, quality initiatives, and staff meetings as required.
- Perform other related duties assigned by direct supervisor.
