Under the supervision of the Program Manager, the Community Health Worker (CHW) Program Coordinator assists in planning and implementing CHW-led intervention activities that are based at Sinai Health System and/or the community. The coordinator works collaboratively with program teams (program manager, CHWs, CHW supervisors, research specialists and others) to engage with community partners and/or Sinai Chicago departments to achieve goals, build/maintain community and Sinai Chicago relationships, and assist with developing intervention protocols, health promotion/educational materials, and processes to measure and track indicators.
They provide input on survey instruments and evaluation tools and assist the program manager with monitoring and documenting activities. The CHW Program Coordinator provides ongoing mentorship (i.e., support, guidance) and training to community health workers, and maintains a caseload, working with patients and community members individually or in groups, based on the intervention.
- Supports the development of CHW interventions and projects, establishing protocols, ensuring patient confidentiality, integration with Sinai Chicago and EPIC (when applicable), data collection, and building and maintaining relationships with clinical teams, community partners, referral sources and other stakeholders important to the success of SUHI and CHW‑lead interventions. Ensures the use of best practices and applies lessons learned from previous projects.
- Coordinates intervention/project activities to meet deadlines.
- Designs and implements outreach strategies to reach targeted audience. Tracks results of different outreach approaches to determine best reach/approach.
- Plans, schedules/organizes, and/or participates in internal and/or external meetings, educational forums, health fairs, and other outreach events important for the success of SUHI and Sinai Chicago’s CHW‑led interventions.
- Monitors the initial and ongoing implementation of protocols for internal and/or community‑based CHW interventions, ensures adherence to protocols, and provides input on modifications as needed. Collaborates with internal team members to creatively solve problems and brainstorm solutions.
- Provides input into the development and tracking of data, data collection and evaluation tools, providing expertise about the CHW role in documentation and best practices for CHW data collection.
- Understands project timeline and expected deliverables. In collaboration with the program manager, monitors progress made toward meeting benchmarks, tracks performance and troubleshoots barriers to goals.
- Documents program successes, including collecting patient and community stories on an ongoing basis.
- Participates in marketing new CHW positions and provides input on the interviewing and hiring of new CHWs, collaborating with internal departments and/or external partners as needed.
- Provides hands‑on support with training new CHWs, facilitates classroom or on‑the‑job training and orientation of new CHWs to their role, protocols and integration within Sinai Chicago departments or community‑based interventions. May include organizing and coordinating initial or ongoing trainings with CROWD staff or other training partners.
- Mentors (and with increasing experience, may supervise) CHWs, plans and conducts meetings as necessary with CHWs and identifies areas of needed training. Consults on difficult cases.
- Maintains a caseload, working with patients individually or in groups; assesses patients/participant needs, provides education, identifies and navigates to healthcare and other resources, provides follow up and communication with clinical teams and on service referrals.
- High school degree or GED. Some college or a degree is preferred but not required.
- Over three years in a community health worker role, with demonstrated leadership and mentorship experience for two years and progressive responsibilities in program coordination for one year.
