Position: House Manager (Housing / Front Door Diversion)

Overview

Schedule: Monday - Friday; 8:00 am - 4:30 pm

Location: East Chatham

The Front Door Diversion Program supports and houses individuals who would otherwise have been referred to a nursing home level of care at discharge from psychiatric hospitalization. The House Manager provides oversight and takes responsibility for clients living in the Front Door Diversion (FDD) temporary unit facility during their shift and collaborates with the Team Leader and Manager in addressing needs of the FD program and clients.

They provide direct services to clients to assist with independent living skills, crisis management, medication management, and symptom management. The House Manager communicates essential client information to their supervisor, providers, specialists, Trilogy staff involved in the client’s care, and residential employees during their shift. They develop relationships with clients to work toward permanent housing and provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance use, utilizing a harm reduction approach.

The House Manager is also responsible for administrative duties in the facility including basic office organization, coordination with building management, and unit turnover for new clients.

Responsibilities
  • Work directly with clients on site regarding case management needs, training in independent living skills, and provide individual/group services or crisis intervention as needed.
  • Provide recovery-focused clinical services to residential clients to assist with independent living (i.e., personal hygiene, housekeeping skills, unit inspections, nutrition, cooking, grocery shopping, laundry, etc.) with the goal of developing tenancy skills to support client transition to permanent housing.
  • Utilize motivational interviewing skills and a harm reduction philosophy of care to engage clients around their current substance use as needed.
  • Model for clients good communication skills, recovery-oriented living skills, effective coping skills, and self-help strategies.
  • Assist clients in identifying signs and symptoms of de-compensation. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
  • Provide de-escalation and crisis management for clients during the shift.
  • Provide psychoeducation, medication training and monitoring to clients according to Trilogy policy.
  • Coordinate with team nurse/provider/pharmacy to ensure client medication accuracy.
  • Coordinate with Housing Department and building management / maintenance to ensure upkeep of all units and ensure successful unit turnover between clients.
  • Maintain documentation and charts to ensure compliance with Medicaid, CARF, IDHS-DMH, and organization standards.
  • Ensure the completion of routine safety drills in accordance with policies, procedures, and guidelines.
  • Assist clients with completing budgets to build and maintain positive financial choices.
  • Document all services provided to clients in electronic health record during shift.
  • Educate and assist clients with applying for entitlements as needed (e.g., SSI, SSDI, Medicare, Medicaid, and LINK).
  • Attend team meetings to collaborate and develop plans for supporting clients with recovery and home needs.
  • Attend department meetings to stay compliant with team updates and trainings.
  • Perform other related duties and/or projects as assigned.
Qualifications
  • Must have one of the following requirements:
    • High School Diploma or GED and two years of supervised clinical experience in a mental health setting.
    • Associate’s or Bachelor’s degree in any field.
  • Must be 21 years of age and demonstrate all of the following:
  • Skill in the delivery of rehabilitative services to adults
  • The ability to work within a provider agency's structure and accept supervision; and
  • The ability to work constructively with individuals receiving services, other providers of service, and the community; or an individual qualified as a Peer Support Worker
  • Experience working with people with mental health and/or substance use diagnoses preferred
  • Experience in residential-based harm reduction services preferred
