Summary
The Medical Case Manager works with clients to provide a broad range of medical case management services and interventions, including home visits. This position works with clients to develop appropriate case plans and goals, and provides ongoing assessment and support. The Medical Case Manager also assists clients in utilizing community resources to meet their individual needs.
Responsibilities- Conducts and engages in outreach and intake services.
- Conducts initial intake interviews and comprehensive assessments; solidifies professional relationships with clients to provide medical case management services.
- Develops case management plans to assist clients in addressing and determining the nature and degree of such problems as personal and family adjustments, finances, employment, food, clothing, housing and physical or mental impairments.
- Counsels clients individually, establishes appropriate service and treatment plans, reviews case progress and determines case closure.
- Provides a range of client-centered services and linkages related to health care, clinical, psychosocial and other support for clients living with HIV/AIDS who are identified as having challenges accessing and maintaining adherence to health care services.
- Follows up with client medical treatments; ensures timely and coordinated access to medically-appropriate levels of health and support services through ongoing assessment of the client’s needs and personal support systems.
- Provides treatment adherence education and counseling to ensure client readiness for and adherence to complex HIV/AIDS treatments.
- Participates in staffings and encourages client involvement in the process.
- Promotes and recruits participation in the 340B prescription plan, and facilitates enrollment for qualified clients as appropriate.
- Maintains updated information on changes in public benefits programs and/or laws that are relevant to clients.
- Completes appropriate client notes, activity forms, medical observations and other required records and reports in order to demonstrate case management work and service hours to agency and regulatory offices; provides timely documentation in agency database.
- Provides necessary client information to Program Manager and across programs in order to coordinate services to clients.
- Participates in required staff meetings and in-service training in order to increase knowledge related to client services, remain informed of agency policies and procedures, coordinate effective teamwork and share professional support; also participates as required in external trainings. This includes a minimum of 12 hours of in-service training in relevant provisions of services to persons with AIDS, sensitivity training, updates on information relating to treatment procedures of HIV/AIDS and training on Health Families America intervention model.
- Utilizes opportunities for direct supervision and consultation in order to facilitate learning and professional accountability within the organization.
- Completes and demonstrates understanding of necessary case management training and procedures.
- In compliance with Children’s Place policy and state and federal regulations, reports immediately to Program Manager any existing or potential abuse of any client as well as any impairment of a parent that would negatively affect the care of the client.
- Informs Program Manager of any change in or concern about the medical condition or status of any assigned client or parent in order to maintain proper primary care.
- In order to provide an environment in which clients and their families can self-determine and seek out support when needed, develops clinically appropriate relationships with clients and guests while maintaining professional boundaries regarding exchange of personal gifts, money, photographs, transporting clients, etc.
