Clinical Supervisor
The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performed.
The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient’s care.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIESResponsibilities of the home health aide include, but are not limited to, the following:
- Assist clients with personal care activities and provide companionship tasks in accordance with the care plan.
- Provide assistance with daily activities such as bathing, dressing, and grooming.
- Assist with mobility and transfers.
- Monitor vital signs and report any changes to the healthcare team.
- Maintain accurate medical documentation and records.
- Perform simple procedures as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).
- Meet the safety needs of patients and using equipment safely and properly (footstools, side rails, etc.).
- Participate as a member of the interdisciplinary team.
- Adhere to the Organization’s documentation and care procedures and standards of personal and professional conduct.

Requirements- Previous experience as a Home Health Aide with current HHA Certification.
- Knowledge of basic medical terminology and procedures.
- Familiarity with medical documentation and record keeping.
- Ability to follow care plans and instructions from healthcare professionals.
- Excellent communication skills, both verbal and written.
- Compassionate and patient demeanor.
- CPR Certification.
- Familiarity with medical software and equipment.
- Ability to build rapport with patients.
- Ability to thrive in a fast-paced environment.
- Well Sky EMR knowledge preferred, but not required.
The home health aide will not function in any manner viewed as the practice of nursing according to the State's Nurse Practice Act. Specifically, the home health aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.
