Social Service Director

As a key member of our leadership team, you will play a crucial role in shaping the social service department's strategy, operations, and outcomes.

• Develop and implement social service programs and services to meet the needs of our residents and their families
• Collaborate with interdisciplinary teams to design and deliver patient-centered care plans that address the social, emotional, and psychological needs of our residents
• Lead and supervise a team of social workers, counselors, and other social service professionals
• Develop and manage budgets for social service programs and services
• Ensure compliance with regulatory requirements and industry standards for social service practices
• Foster strong relationships with community partners and stakeholders to enhance our social services and advocacy efforts
• Monitor and evaluate program outcomes, identifying opportunities for improvement and implementing necessary changes

Minimum 2 years of experience in social work, case management in Skilled Nursing.
