Responsibilities
  • Meets directly with patient/family to perform a comprehensive psychosocial assessment including social, emotional, cultural, mental status, environmental and financial circumstances; recommends a plan of intervention based on mutually established goals.
  • Conducts and documents initial psychosocial assessment, appropriate interventions, and expected transition in a comprehensive, clear, timely and legible manner, addressing reactions to illness, adjustment to medical setting, and linkage to community resources.
  • Facilitates health care surrogacy/guardianship processes for patients deemed non-decisional; leads efforts to identify family or surrogate decision makers; collaborates with service lines and legal to file for state guardianship when necessary.
  • Coordinates groups for supportive interventions or educational opportunities; addresses financial issues related to insurance coverage and payment; refers to Financial Case Management.
  • Manages conflict resolution and complex family dynamics impacting plan of care and discharge planning; coordinates complex clinical case conferences.
  • Investigates, manages, and reports suspected abuse/neglect of minor children and vulnerable adults; facilitates involvement of IDCFS and IDOA; identifies, reports, provides intervention, offers safety planning, and documents cases of suspected DV; involves police as needed.
  • Assesses for depression and suicidal ideation using validated tools; addresses psychiatric symptoms and substance use disorders.
  • Provides supportive care for patients considered for hospice/palliative care and participates in family meetings to discuss goals of care and end-of-life planning.
  • Addresses needs for unhoused patients; performs assessments of the physical environment and adequacy of support systems to prevent crisis or hospitalization.
  • Projects include Social Work-related tasks such as participation in Quality Improvement committees and other committees; engage in coverage, training, and clinical performance activities as assigned.
  • Provide cross-coverage to other clinics and inpatient units as needed; assist with training new staff and student interns.
  • Assist in the collection and reporting of financial indicators (LOS, avoidable days, resource utilization, discharge barriers, cost per case, readmission rates, denials); use data to drive decisions and plan/implement performance improvement strategies for assigned patients/units.
  • Collect data for discharge delays and other performance/outcome indicators; participate in tool development, implementation, evaluation, and revision with the healthcare team.
  • Maintain professional development and social work education requirements; participate in workshops, conferences, and in-services.
  • Develop and lead groups or education sessions as needed; perform functions appropriate to the role of a Social Worker.
  • Maintain competency for Licensed Social Worker (LSW) and Clinical Licensure (LCSW) and credentialing to support revenue generation where applicable.
