Activities:

• Assists RN to assign patients to beds based on Sinai Chicago’s established priorities for bed assignment and other bed assignment indicators to minimize bed down time, maximize the clinical usefulness of each bed day and meet the convenience of patient.
• Monitors and orders supplies for department.
• Assists RN to provide routine feedback to admitting when there are issues of accuracy, completeness, or lateness of delivery.
• Rounds on nursing units for status of bed availability, discharges, place patient names in ADT book. Assists caregivers with movement of patients as needed.
• Assists the RN to track the status of beds and patient waiting time in the EDs, external hospital transfers, direct admits from home/clinic, and procedural areas.
• Assists the RN with gathering pertinent clinical information from external hospitals to determine which service line and provider to contact for acceptance.
• Obtains accurate customer demographic information and place in the EPIC transfer software for nursing staff.
• Assists with patient transportation coordination.
• Answers incoming telephone calls for Sinai Chicago Patient Transfer and Navigation Center in a pleasant, professional manner. Identifies customer needs and determines how to assist in the appropriate manner.
• Coordinates outbound transfer follow up and communication.
• Contacts patients with bed number and direction for planned admissions.
