Hybrid: Two Days Onsite, Three Days Remote
The Service Coordinator works with families located in Chicago's Northside neighborhoods, up to and including Evanston and will be required to travel to these locations.

Pay Range: $20.19 - $31.80 per hour

SummaryThe Service Coordinator I with Child and Family Connections assists in determining eligibility for designated services and enables families to receive their rights, procedural safeguards, and services that are authorized under the state’s early intervention program. The Service Coordinator builds relationships that include providing information to families and coordinating information among multiple service providers. The service coordinator maximizes the use of existing resources and refers families to any other needed services, tailoring the unique circumstances of each individual child and family.

- Bachelor’s degree in a related human service field such as early childhood special education, social work or psychology.
- Must meet requirements to obtain Illinois Department of Human Services Part C Early Intervention Service Coordination Credential.
- Successful completion of state-sponsored Service Coordinator, Systems Overview, and Cornerstone trainings are required.
- Ability to gather information, format reports, and present outcome of work assigned.
- Interpersonal skills are integral to the position and the ability to coordinate activities with many disciplines toward one goal is essential.
- Case management experience is essential.
- Good time management skills and the ability to work independently are required.
- Demonstrates knowledge of the developmental, physical, psychosocial, medical and cultural needs of children and families.
- Demonstrates excellent organizational and case management abilities.
- Ability to work with different cultures and aid family with various barriers to care is essential.
- Physical: able to be active and travel. Use of personal car required.
- Mental: able to understand complex diagnosis and treatment information and use deductive reasoning.
- Social: collaborates and supports the objectives of the Child and Family Connections grant.
- Knowledge of the developmental, physical, psychosocial, and cognitive development of children 0-3 years is preferred.
- Basic knowledge of typical/atypical child development is preferred.

Responsibilities- Provides direction to families on the early intervention system in the state and local area, including their rights and availability of services.
- Assists families in identifying available service options, including the availability of advocacy services.
- Gathers all pertinent medical, developmental and social information for the child and family and coordinates the performance of evaluations and assessments in a timely manner.
- Facilitates and participates in the development, review, evaluation and maintenance of Individualized Family Service Plans (IFSP).
- Coordinates and monitors the ongoing delivery of services to ensure that all resources are being utilized and the IFSP goals are being addressed.
