This Opportunity is for "PRN" variable days of the week
Evaluates and treats patients using the principles and practices of physical therapy and in accordance with the patient’s age, developmental level, and diagnosis. Collaborates with the interdisciplinary team. License-eligible therapists would perform duties under the supervision of a licensed therapist in accordance with the Illinois Physical Therapy Practice Act.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Evaluate Out Patients, establishes goals, and develops individualized treatment plans as prescribed.
- Provides direct patient treatment plan based on the patient's response.
- Provides education to patient regarding patient’s abilities/assistance for therapeutic intervention.
- Recommends modification or changes in the patient's treatment program to physician.
- Prepares written/computer documentation as required by the profession.
- Participates in the interdisciplinary patient care conferences and rounds a care plan.
- Communicates with other professional staff and patient's family/support system.
- Arranges work schedule to make efficient use of direct and non-direct patient care time.

Education and/or Experience- Must have 1+ years of experience in acute care
- A Bachelor’s degree is required
- A Master’s and/or Doctorate Degree in Physical Therapy is preferred
- EMR/EPIC experience preferred
- Current Illinois license in Physical Therapy
