We’re looking for an Occupational Therapist passionate about helping older adults & individuals with disabilities thrive safely & independently at home. A successful candidate is highly empathetic, can juggle client, program & business needs, and is eager to make a lasting impact. Comfort working with new systems & tools is an advantage; a plus if home modification work is part of your experience.

  Contracted, minimum 10 hrs weekly

  In‑Home Visits

• Conduct in‑home assessments & functional evaluations
• Deliver personalized, evidence‑based recommendations for home modifications
• Educate clients & caregivers on fall prevention & safe mobility techniques
• Engage clients & families in collaborative care planning
• Work closely with Director of Clinical Operations to share findings & support process improvement initiatives
• Maintain accurate, timely documentation via electronic system

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging‑in‑place, or disability services preferred
• Strong interpersonal & clinical reasoning skills
• Reliable transportation & valid driver’s license
• Proficiency with technology & digital documentation
• Ability to work independently while collaborating with remote team

• Contractors are paid monthly, within 2 weeks of the previous month’s end.
• We pay per completed evaluation, which includes:
  • Travel to/from client’s home
  • The evaluation itself
  • Documentation
• If needed, we will pay additionally for extended travel after prior discussion & approval.
