Responsibility Summary

The Family Support Specialist provides comprehensive case management and support services to caregivers of child victims of sexual abuse and their families, with the goal of helping families to engage and remain in mental health services.

Consultus receive referrals directly from family advocates, Family Hope Center therapists, or the Director of PATHH and Family Support Services. Referrals include medical advocacy, mental‑health linkage, basic needs, education advocacy, psychiatric services, or application for benefits (e.g., Social Security, DHS, All Kids, WIC).

Primary Responsibilities
  • Support the Director of PATHH and Family Support Services in maintaining the Centralized Wait List by providing ongoing follow‑up to families waiting for mental health services.
  • Provide support to the Family Hope Center Drop‑In Service program.
  • Assess client needs, complete a Wheel of Life, and develop a service plan that meets clients and family goals.
  • Update service plans and complete Wheel of Life with all active clients quarterly.
  • Facilitate 2–4 therapy or psycho educational groups per year (child, parent, combined, or family).
  • Participate in weekly reflective, clinical, and administrative supervision with the Director of PATHH and Family Support Services.
  • Participate in Family Hope Center file reviews and department meetings and case consultations.
  • Provide supportive case management based on the child and family’s needs and the service plan goals.
  • Facilitate linkages and referrals to meet identified resource needs.
  • Coordinate services with internal and external providers to achieve service goals.
  • Attend appointments with families when support for self‑efficacy and self‑esteem is needed (e.g., housing, psychiatry, school, employment).
  • Accompany families to court proceedings for support, translation, and education.
  • Conduct risk assessments of children and families per Chicago

    CAC policy and provide ongoing assessments as necessary.
  • Collaborate with clinicians to gather collateral information (e.g., DCFS, CPD, Advocate, FI, Psychiatry, school).
  • Gather data on service referrals, linkage success, client participation, and satisfaction.
  • Plan, facilitate, and participate in Family Hope Center community events and foster a supportive environment.
  • Communicate issues, concerns, and successes in a timely manner to adjust case management services.
  • Participate in all team and agency‑wide meetings.
  • Maintain a flexible work schedule as required by case needs.
  • Participate in local networking and training opportunities.
  • Perform additional duties as assigned.
Discretion

The Family Support Specialist has a low-level range of discretion and independent decision‑making.

Supervisory Responsibilities

The Family Support Specialist does not have supervisory responsibilities.

Relationships

The Family Support Specialist works closely with the Director of PATHH and Family Support Services, Family Hope Center team, and the advocacy team.

Qualifications

• Master’s Degree in Social Work, Counseling, or a related Human Service field (licensure preferred).

• Familiarity with Chicago’s mental health community and social service agencies.

• Minimum of 2 years’ experience in sexual‑abuse treatment, child welfare, and/or case management.

• Strong clinical, written, communication, and organizational skills.

• Bilingual in English and Spanish.

• Ability to work independently in a fast‑paced collaborative environment.
