PRN Physical Therapist, Outpatient Therapy in the Home – Chicago/South Suburbs
The Physical Therapist provides outpatient quality physical therapy within patients' homes and serves as a liaison for physician practice to meet therapy needs. The role offers high flexibility and autonomy, allowing a focus on individualized care and growth of a new service line.
Responsibilities- Provide one-on-one in-home therapy for post-operative patients.
- Devise and execute detailed treatment plans.
- Maintain positive and respectful working relationships with clients, employees, physicians, and family.
- Work independently while providing empathetic patient care.
- Build relationships with patients and the community.
- Communicate continuously with referring physicians and patient families.
- Maintain patient charts, clinical logs, and compliance checklists.
- Set up and monitor home exercise programs.
- Provide education specific to diagnosis and symptom management.
- Respond to priorities in patient care with initiative and sound judgment.
- Demonstrate willingness to adapt workload and assist with training/orienting others.
- Stay abreast of and comply with state and federal laws including HIPAA, ADA, OSHA, and FLSA.
- Other projects as assigned.
- Bachelor’s Degree from an accredited Physical Therapy Program.
- Minimum one-year experience in an outpatient setting preferred.
- Current State of Illinois Physical Therapy license.
- Current CPR Certification.
- BCLS or ACLS.
- Valid Driver’s License, reliable transportation, and required liability insurance for home visits.
USD $80,000 - $100,000 annualized. Compensation determined by many factors, including skills, experience, education, certifications, geographic location, market data, and internal equity. Base pay is only one portion of the total rewards package.
