JOB SUMMARY
As a Hospice Aide you will perform personal care activities related to the activities of daily living, under direction of a Case Manager [RN].
QUALIFICATIONS- Must have reliable vehicle.
- 12 months of caregiving experience required (home care, assisted living/memory care, skilled nursing/nursing home, hospital or hospice).
- Hospice experience preferred.
- Excellent customer service skills.
- Current CPR Certified.
- Valid driver's license and auto insurance.
- Personal cares (bathing, dressing, grooming, toileting, skin care, etc).
- Review patient care plan.
- Lifting and transferring.
- Assistance with range of motion & positioning, exercise & ambulation.
- Assistance with dining & nutrition.
- Electronic charting.
- Needs strong understanding of professional boundaries.
- Must be able to lift 50 lbs.
- Good understanding of body mechanics.
- Strong communication skills—written, verbal, listening with attention.
- Good problem‑solving skills.
- Good time management of self and others.
- Able to function in an electronic medical record.
