Clinical Pharmacist – OR Pharmacies, Full-time Rotating Shifts
Northwestern Memorial Hospital is seeking a Clinical Pharmacist to provide patient‑centered medication management on a rotating shift schedule. Applicants must hold a current Illinois pharmacy license and a Doctor of Pharmacy degree (or a bachelor’s degree in pharmacy).
Salary Range$61.00 – $82.35 per hour. Placement within the range depends on experience, internal equity, and union guidelines.

The Clinical Pharmacist is responsible for identifying, preventing, and resolving medication‑related problems, providing authoritative information to health care professionals, patients, and the public, and ensuring that the pharmacy services function as an integrated system to provide patient‑centered care.
Responsibilities- Interpret medication orders to ensure safety and proper use, alerting to compatibilities and potential administration errors.
- Monitor medication therapy for significant interactions and intervene when warranted.
- Evaluate and document appropriateness of medication selections and communicate potential problems to prescribers.
- Act as a consultant in pharmacokinetics and adverse reactions, participating in therapeutic drug monitoring programs.
- Process medication order information into the Electronic Health Record, ensuring accurate records.
- Maintain a comprehensive pharmacy patient profile, including age, weight, allergies, and diagnosis.
- Oversee packaging, labeling, and distribution of medications, monitoring controlled inventories.
- Compound and dispense pharmaceuticals, including sterile products and parenteral nutrition.
- Verify medication orders before delivery to patient care areas, resolving discrepancies as needed.
- Review laboratory data for medication appropriateness and recommend changes.
- Develop and present orientation, education, and training programs for staff and pharmacy students.
- Educate patients and families about specific medications.
- Respond to emergency situations involving medication management.
- Maintain the hospital formulary, limiting usage to formulary items when appropriate.
- Document all clinical activities and interventions, collecting data and records as required.
- Communicate professional knowledge effectively to patients and health professionals.
- Participate in quality improvement and medication use review activities.
- Organize, prioritize, and plan workload, supervising technician assignments and ensuring departmental goals are met.
- Review and restock automated dispensing cabinets, resolving any issues.
- Ensure compliance with hazardous waste procedures.
- Maintain proficiency through continuous professional development.
- Perform other duties as assigned.
- Current Illinois pharmacy license
- Bachelor’s degree in Pharmacy or a Doctor of Pharmacy degree
- Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.)
- Completion of a pharmacy residency program or equivalent hospital pharmacy experience
