Responsibilities:
1. Insure menus are posted on all the floors and main dining room. Insure water containers are cleaned and refilled daily.
2. Supervise dining room, hallways and all common areas on the first floor. Insure residents are safe and free of any at-risk situations.
3. Monitor residents on the first floor for conflicts, unsafe behaviors, agitation, aggression, at-risk medical conditions, elopement and any other condition that puts resident at risk for harm. With the onset of at risk-risk conditions the Behavior Coordinator/smoke Monitor will take actions to insure resident safety. This can include and not limited to:
A. Separating residents in a conflict
B. Providing verbal guidance and prompts to residents who exhibit verbally inappropriate behaviors, non-compliance with facility rules and other at—risk behaviors.
C. Redirecting residents to safety who exhibit at risk behaviors.
D. After a behavioral intervention, Behavior Coordinator will complete a Behavioral Tracking form, provide the form to the Director of Social Services and verbally describe the behavioral Intervention and outcome to both Director of Social Services and Director of Nursing.
4. During scheduled smoking sessions the Behavioral Coordinator/Smoke Monitor will take residents who desire to smoke to the outdoor smoking area. Behavior Coordinator/ Smoke Monitor will:
A. Distribute cigarettes to residents.
B. Light residents’ cigarettes.
C. Instructs residents on safe smoking behaviors.
D. Monitors and provides interventions for any at risk behaviors.
E. Directs residents back in to building at the end of smoking time and when there is a behavioral incident, completes a Behavioral Tracking Form, provide the form to the Director of Social Services and verbally describe the behavioral intervention and outcome to the Director of Social Service and Nursing.
5. Behavior Coordinator/Smoke Monitor will be available to all areas of the building to provide support and intervention when staff identify a behavioral incident.
6. When the Behavior/Smoke Monitor is called to another part of the building for a behavioral incident he/she will do the following:
A. Via walkie /talkie will inform Resident Service Coordinator and Social Service staff that he/she is leaving floor and going to another location.
B. Behavioral Coordinator/Smoke Monitor will immediately go to the floor where the behavioral incident is occurring.
C. Once on the unit of the behavioral incident the Behavioral Coordinator/Smoke Monitor will provide the guidance and interventions to effectively address the behavioral incident. Behavior Coordinator will complete a Behavioral Tracking Form and provide the form to the Director of Social Services and verbally describe the behavioral Intervention and outcome.
7. Behavior Coordinator/Smoke Monitor will work closely with the interdisciplinary team and report directly to the Social Service Director/ or Designee
