MDS Coordinator

Aperion Care International, a skilled nursing and rehabilitation facility in Illinois, is seeking an experienced and detail-oriented MDS Coordinator to join the clinical leadership team. This role is ideal for a nurse who is highly organized, knowledgeable in MDS/PDPM, and committed to ensuring accurate resident assessments and strong clinical outcomes.

• Complete and manage the full MDS/RAI process in accordance with state, federal, and CMS regulations.

• Coordinate interdisciplinary assessment schedules and ensure timely submission.

• Oversee care plans and ensure accuracy of clinical documentation.

• Collaborate closely with nursing, therapy, and administrative teams to support resident outcomes and reimbursement accuracy.

• Participate in quality assurance, survey preparation, and performance improvement initiatives.

• Provide education and support to staff regarding MDS and clinical documentation requirements.

• Active RN license in Illinois (required).

• Prior MDS Coordinator experience in a skilled nursing facility.

• Strong knowledge of PDPM, RAI guidelines, and LTC documentation standards.

• Excellent communication, assessment, and organizational skills.

• Ability to work independently while collaborating with the interdisciplinary team.
