Home Care Provider

Base pay range: $18.00/hr - $18.00/hr

Position Summary

This position provides in‑home care services to older adults, according to established plans of care and in accordance with the Illinois Department on Aging (IDOA). Under the supervision of an assigned supervisor, the caregiver provides assistance with activities of daily living and personal care including bathing, grooming, grocery shopping, and other essential errands; performs routine housekeeping and laundry; assists with meal planning and preparation;

and provides self‑administered medication reminders.

Essential Duties and Responsibilities Participant Service Delivery
  • Assist participants with meal planning and engage in meal preparation as needed.
  • Perform routine light housekeeping tasks such as making and changing beds, dusting, washing dishes, vacuuming/sweeping, cleaning/mopping floors, and doing laundry.
  • Assist participants with local grocery shopping and other essential errands, maintaining required receipts and documentation.
  • Provide self‑administered medication reminders, including reading instructions and uncapping medication containers.
  • Assist with personal care tasks (shaving, hair washing, bathing, dressing, oral hygiene, transferring, and range‑of‑motion exercises).
  • Follow written special‑diet plans and reinforce dietary recommendations as indicated.
  • Accompany participants to appointments via public transportation or community sources (taxicab, Lyft, Uber, or Medi-car) when required by the plan of care.
  • Maintain service schedule records and daily calendars.
  • Monitor participants’ progress and report any concerns, changes, or issues regarding safety and well‑being to the supervisor in a timely manner.
  • Maintain positive, professional relationships with participants and families and protect confidentiality of personal information.
  • Perform other duties as assigned by a supervisor or director.
Performance & Quality Management
  • Participate in the agency’s Performance Quality Improvement process and support PQI activities.
  • Incorporate the agency mission, philosophy, and core values into daily work and decision‑making.
  • Adhere to agency policies, including Universal Precautions, Exposure Control Plan, and mandated reporting of abuse or neglect.
  • Report situations that pose a threat to staff, participants, or the agency according to incident and IDOA critical‑event procedures.
  • Attend staff meetings, monthly in‑service training, and regular individual supervision.
  • Submit timesheets and remain accountable for activities within approved agency activities.
General Requirements
  • Must be at least 18 years of age.
  • Demonstrate sensitivity to cultural diversity.
  • Subject to drug and alcohol screening, aptitude, and skills testing.
  • Capable of performing all essential duties satisfactorily.
  • Ability to work independently and collaboratively with diverse populations.
  • Willingness to perform 24 hours of initial pre‑service training and 12 hours of annual in‑service training.
  • Basic knowledge of home‑management skills.
Education and Experience
  • Minimum of a high‑school diploma or GED (strongly preferred).
  • At least one year of documented experience as a homemaker, caregiver, or childcare provider.
  • Experience working with older adults preferred.
Technology Requirements
  • Must have and use a smartphone (iPhone, Android, Blackberry, Google, or Windows phones) for essential software applications.
  • Flip phones and tablets (including iPad and Android) are not allowed.
  • Required to download and access specific business‑related applications used by the Home Care Services program.
Language Skills
  • Bilingual in Spanish and English strongly preferred.
Computer Literacy and Knowledge Skills
  • Basic computer skills a plus.
Mathematical Skills
  • Basic addition and subtraction.
Operating Environment and Working Relationships
  • Work independently as needed.
  • Interact regularly with coworkers, participants, supervisors, and families.
  • Demonstrate tact, professional composure, and diplomacy.
  • Show patience and a supportive demeanor, especially with participants with higher needs, cognitive limitations, or agitated behavior.
