Care Coordinator – Erie Family Health Centers

Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Social Determinants of Health (SDOH) team. The Care Coordinator assists patients onsite at the clinic and collaborates closely with the clinical care team to connect Erie patients with both internal and community resources, aimed at enhancing their overall health and well‑being.
MainDuties & Responsibilities
- Screen patients for needs around SDOH and provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.
- Enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.).
- Educate and empower patients to navigate future SDOH needs.
- Distribute and track SDOH resources including Top Box Food distribution, ordering Uber Health rides, and distributing other material resources.
- Use Epic to document work, respond to referrals and create SDOH panels and outreach risks.
- Build effective relationships with local social service agencies and advocate for clients in the process.
- Perform other duties as assigned.
- Education
- High School Diploma or equivalent required.
- Associates degree or Bachelor’s degree preferred.
- Skills and Knowledge
- Minimum of two years of work experience in community health, social or community services, or comparable experience preferred.
- Bilingual fluency (English/Spanish) preferred.
- Local travel required.
- Intermediate proficiency in MS Office preferred.
- Verbal and written communication skills, including high level of comfort presenting to groups.
