Purpose of Your Job Position

The primary purpose of your position is to assist the Charge Nurse in the provision of nursing care to residents as assigned and under the direction of the Director of Nursing. All services provided shall be in accordance with established nursing standards, policies, procedures, and practices of this facility and the requirements of this state.

Duties,

Responsibilities and Essential Functions Administrative Functions
  • Report promptly and as scheduled to Charge Nurse for shift report.
  • Perform all assigned tasks in accordance with established policies and procedures and as instructed by your Charge Nurse.
  • Ensure that residents/families receive the highest quality of nursing service and help provide a caring and compassionate atmosphere at all times.
  • Follow and complete all resident care assignments.
  • Report all uncompleted assignments to Charge Nurse.
  • Cooperate with inter‑departmental personnel (activities, housekeeping, laundry, dietary, etc.) to ensure that all services are adequately maintained and provided in order to meet the needs of the residents.
  • Attend all scheduled training programs including initial orientation as well as annual and required training programs.
  • Follow established procedures for accurately identifying residents prior to administering care and treatment.
  • Ensure the timely and prompt reporting of all resident‑related accidents, incidents, falls and changes in condition to your Charge Nurse.
Infection Control
  • Follow all Infection Control Protocols and Universal Precautions.
  • Follow all established isolation precautions, policies and procedures.
  • Follow all established procedures for the use and disposal of personal protective equipment and resident items.
  • Wash hands before and after performing all services for any resident.
Nursing Care
  • Ensure that all residents have access to call lights throughout each shift.
  • Monitor all residents who are unable to call for help.
  • Report all changes in resident conditions to Charge Nurse as soon as possible.
  • Check assigned residents frequently during each shift to ensure their needs are being met in a timely manner.
  • Clean, disinfect and return all resident care equipment to designated storage area.
  • Clean and disinfect resident wheelchairs, geri chairs, walkers and other equipment as needed and as assigned.
  • Assist residents with activities of daily living (ADL) as assigned and whenever needed including but not limited to hair care, shaving, nail care, dressing, undressing, etc. (as assigned or as necessary).
Nursing Care
  • Monitor incontinent residents in a timely manner to ensure residents are clean and dry.
  • Actively participate in pressure ulcer prevention and reduction programs.
  • Assist residents with bowel and bladder functions in a timely manner (i.e. take to bathroom, assist with bedpan, urinal, or portable commode).
  • Assist residents as assigned with bath and shower functions. Consult with Charge Nurse for appropriate interventions for residents who refuse or are resistant to bathing.
  • Assist and perform all lifting, turning, moving, positioning, and transporting of residents consistent with procedures and policies.
  • Wear and use safety and lift equipment per established policy and procedure.
  • Weigh and measure residents as instructed and assigned.
  • Operate all equipment in a safe manner and per policy and procedure. Use only equipment you have been trained to use.
  • Report all defective equipment promptly to Charge Nurse.
  • Inform Charge Nurse whenever additional supplies and equipment are needed to perform resident care.
  • Refer to resident Care Cards to determine daily care needs and to assist in the assessment of changes in resident conditions.
  • Document all resident care in the medical record consistent with facility policies and procedures.
