Are you an Occupational Therapist (OT) who’s passionate about helping older adults live fuller, more independent lives? Our Symbria Rehab Team at Plum Creek is looking for its next employee‑owner — someone who’s ready to make a meaningful difference every day.

As a PRN Occupational Therapist (OT), you will establish appropriate goals and creatively execute the treatment plans along with supervision of the Occupational Therapist Assistants. As a member of our interdisciplinary team, you will be required to maintain productivity standards and complete required documentation. More importantly, you will help your patients achieve critical outcomes while acting as a trusted partner to the community you serve.

• Develop and implement creative, individualized treatment plans
• Supervise and collaborate with Occupational Therapy Assistants
• Work as part of an interdisciplinary team to achieve patient‑centered outcomes
• Maintain documentation and productivity standards with accuracy and care

• Graduate of an accredited Occupational Therapy program
• Valid state OT license (or pending for new grads)
• Flexibility to work varied hours, including weekends
• Experience in acute care, long‑term care, or rehab preferred — not required!
