Sign‑on Bonus: $1,500.
Schedule: Monday – Friday, 8:00 am – 4:30 pm.
Location: South State.
The Recovery Counselor evaluates each client’s personal, medical, emotional, social, and environmental situation to design a treatment plan via home, community, and office visits. They provide client‑centered, strengths‑based, trauma‑informed services to individuals with severe mental illnesses and co‑occurring substance abuse disorders, using a harm‑reduction approach. The counselor advocates for clients, links them to community resources, documents all services per agency policy and state requirements, and provides crisis/on‑call coverage as directed.
Responsibilities- Assess clients’ personal, medical, emotional, social, and environmental situations through home, community, and office visits.
- Coordinate the development of an individualized recovery plan with the client, support system, and other care providers.
- Facilitate education on substance use, misuse, and risks associated with dual diagnosis of mental health and substance issues for clients and team members.
- Provide one‑on‑one services focused on substance use goals.
- Acquire a SAS credential within 12 months of hire.
- Identify opportunities to move clients to appropriate levels of care, provide referrals to additional services, and link individuals to community substance‑use treatments as needed.
- Assist clients in recognizing signs of de‑compensation and develop coping skills to manage symptoms and stressors.
- Support clients in learning and improving independent living skills such as personal hygiene, housekeeping, nutrition, and shopping.
- Educate and help clients apply for entitlements (SSI, SSDI, Medicare, Medicaid, and LINK).
- Accompany clients to important community appointments and provide support related to substance use, housing, budgeting, social support, and medication.
- Advocate on the client’s behalf and empower clients to advocate for themselves when appropriate.
- Maintain effective communication within a multidisciplinary team model for client care.
- Perform other related duties and/or projects as assigned.
- Bachelor’s degree or high‑school diploma/GED plus two years of supervised clinical experience in a mental‑health setting.
- Minimum one year of experience working with clients with substance‑abuse issues.
- Valid Illinois driver’s license with daily access to a well‑maintained vehicle and $100,000/$300,000 liability insurance.
- Relevant certification (CADC, CRADC, CSADC, CAAP, or CARS) through the Illinois Alcohol and Other Drug Abuse Professional Certification Association, or willingness to obtain within 6 months of hire.
- Ability to work evenings and weekends as directed.
- Experience working with people with mental‑health diagnoses is preferred.
- Experience in community‑based services is preferred.
