Social Worker LSW/LCSW Behavioral Unit Registry Inpatient Responsibilities
- Complete a comprehensive bio-psychosocial assessment of patients and families; evaluate information; select appropriate Social Work methods; develop, implement, and evaluate a plan in collaboration with Case Manager and multidisciplinary team.
- Actively participate in length of stay meetings, discharge planning rounds, unit daily reports, clinical practice teams, and department meetings.
- Initiate patient/family and provider team meetings to develop strategies related to psychosocial and economic issues, particularly those concerning care progression and transition; serve as liaison between patients/families and all providers of care within and outside the organization on social issues.
- Lead the process of obtaining legal guardianship and competency determinations; complete and disseminate all required legal and clinical documentation.
- Facilitate patient appointment setting and address perceived barriers to care; provide daily follow-up communication for referrals, needs assessment, and assistance in making appropriate healthcare decisions for patients seen during non-coverage hours.
- Monitor patient length of stay in coordination with Utilization Review Department; initiate discharge planning within first 24 hours of admission; alert department management of potential length of stay issues.
- Participate in the development of effective treatment plans for assigned cases; present summaries of social service assessments, involvement, and recommendations at multidisciplinary patient staffing meetings.
- Be knowledgeable about psychiatric disorders and the population served in Behavioral Medicine; develop individualized treatment plans; perform individual counseling and group therapy.
- Provide leadership for group therapy; document patient participation and progress in progress notes once per week.
- Provide family assessments and family therapy when ordered by the psychiatrist.
- Assist in milieu management; work with other staff on rule and limit setting within the unit, including de-escalation skills.
- Licensed clinician LSW or LCSW.
- BLS Certified.
- Master’s Degree in Social Work (most candidates hold this degree).
- Bilingual Spanish/English a plus.

Compensation range: $22-$30 hourly.
