Join Our Team as a Family Medicine Physician!

Are you passionate about providing comprehensive care to patients with complex and chronic medical conditions? At Absolute Care Medical Center & Pharmacy in Chicago, we are looking for a dedicated Family Medicine Physician to join our multidisciplinary team. Here, you’ll play a vital role in managing transitions across care settings and preventing hospital readmissions, all while practicing evidence-based medicine within a patient-centered medical home model.

As a Family Medicine Physician, you will:

• Deliver Complex Clinical Care: Focus on improving health for patients with multiple chronic conditions.
• Collaborate with a Multidisciplinary Team: Work alongside social workers, case managers, and other healthcare professionals to create effective care plans.
• Manage Transitional Care: Develop and track care plans for patients transitioning from hospitals, ERs, and skilled nursing facilities.
• Treat Acute Exacerbations: Provide immediate care for chronic conditions, ensuring patients receive the right level of treatment.
• Educate Patients: Help patients understand chronic disease management and the importance of advanced directives.
• Practice Evidence-Based Medicine: Uphold the highest standards of care in accordance with the PCMH model.

To thrive in this role, you should have:

• Active medical license in the state of practice.
• Board certification in Internal Medicine or Family Practice.
• BLS certification and DEA registration.
• A mission-driven mindset to serve underserved populations.
• Strong teamwork skills and the ability to build successful relationships.
• Experience with EMR systems and a knack for creative problem-solving.

• Experience in population health management.
• Background in working with psycho-socially and medically complex patient populations.

This position operates in a professional office environment, utilizing general office equipment. You may be required to wear protective attire and will be exposed to various patient conditions daily.

• Ability to communicate clearly and exchange accurate information.
• Capability to remain stationary for extended periods.
• Frequent use of computers and office equipment.
• Occasional physical demands, including moving objects up to 20 lbs.
