- See approximately 3 patients per hour; 80% adults, 20% under 18
- Monday to Friday, 8 am to 5 pm, no call responsibilities
- Work 4 days onsite and 1 day from home; telehealth options available, but at least 1 day per week onsite in Chicago
- Must speak Spanish at a 7/10 proficiency or higher; many patients are older and primarily Spanish-speaking

- Team-oriented, experienced with PCMH model, collaborating with MAs, receptionists, care coordinators, and care managers
