Monday Friday, day shift + only 1 weekend every 6 weeks

Your Role:

Provide one-on-one Physical Therapy services to patients in their homes.
Assess patient functioning levels and develop individualized treatment programs.
Educate patients and caregivers on therapy programs and equipment use.
Supervise Physical Therapy Assistants and collaborate with healthcare teams.
Prepare reports and contribute to agency development initiatives

Qualifications

Master's Degree or Higher in Physical Therapy

At least 1 year of experience in a healthcare setting (No new grads).
Valid Illinois (IL) PT License & Drivers License

Home Health experience preferred, but inpatient rehab experience will be considered

Must have either Home Health or inpatient PT experience; candidates without this will not be interviewed

Monday to Friday, daytime shifts with start & end times varying based on patient needs

1 weekend every 6 weeks (Saturday & Sunday, day shift with scheduled patients)

2 holidays per year (1 summer, 1 winter on a rotating basis)

Local travel required to patients' homes in Elmhurst, Oak Park, Cicero, Maywood, Bellwood, La Grange, La Grange Park & surrounding areas
