Our Self-Management Team empowers patients and their support networks to take control of their own nursing needs wherever possible. With an exceptional success rate, we focus on promoting independence, building confidence, and enabling individuals to manage their care safely and effectively. We work collaboratively with patients to help them develop the skills and knowledge required to take responsibility for their own health needs--supporting better outcomes and improving quality of life.
Previous community experience is essential but we offer a robust local induction tailored to individual needs.
Main duties of the jobThe successful applicant will work under the support and guidance of the District Nurse to facilitate patient self-care and promote independence within their own home environment.
Your responsibilities will include assessing, planning, implementing, and evaluating individualised patient care in a compassionate, safe, and clinically effective manner. Throughout all aspects of your work, you will maintain the highest standards of patient privacy, dignity, and respect.
A key requirement of this role is the ability to travel daily across both the Chesterfield and North East Derbyshire localities to visit patients in their own homes.

Person Specification Education, Qualifications and Training- Qualified RGN
- Evidence of recent learning
- Experience of working in the community
- Evidence of research based/reflective practice
- Knowledge of local and national priorities.
- Understanding of the wider NHS agenda regarding community services
- Long Term Conditions
- Excellent communication and interpersonal skills
- Written and recording skills
- Ability to assess, plan and implement care
- Ability to supervise junior team members
- IT Skills
- Ability to travel across the locality to patients homes
