The post is for a substantive Consultant in Elderly Medicine. The Elderly Medicine department has expanded rapidly, bringing the number of geriatricians to thirteen. The department offers opportunities for service development in comprehensive elderly care. The post supports Acute Frailty services and may involve direct or subspecialty support. The Countess of Chester Hospital NHS Foundation Trust is an RCP Future Hospital exemplar site transforming urgent care pathways, working with community health and social care providers and Western Cheshire CCG to deliver an integrated model.
The Trust has a strong Medical Education department and the post holder will have a significant educational role.
Specialist interest in acute geriatrics required. The post supports an established acute geriatrics team based at Countess of Chester.
Key responsibilities include:
- Regular ward rounds on inpatient frailty ward, supporting junior doctors and the multidisciplinary team. Attend daily MDTs. Named consultant for 14 inpatients plus outliers (indicative 0–4 patients). Approximately 300–350 patient reviews per month.
- General elderly clinic and a subspecialty clinic (subspecialty flexible to candidate's interest: movement disorders, falls, stroke, community). Indicative clinic numbers: 8 new and 32 reviews per month for a weekly 3 hour clinic.
- "Front door" acute geriatrician sessions: support the emergency department, rapid response team and acute physicians with specialty inreach providing early advice and input into care and management plans for frail elderly patients in the acute setting.
- Supervision of junior doctors.
- Attendance to and input into specialty and departmental meetings.
The Countess of Chester Hospital NHS Foundation Trust serves West Cheshire and Welsh patients, collaborating with the wider Cheshire and Merseyside Health and Care Partnership. Services are provided from three locations:
- The Countess of Chester Hospital: 438 general and acute beds
- Ellesmere Port Hospital: 56 beds (rehabilitation, intermediate and outpatient)
- Tarporley War Memorial Hospital: base for community services serving the local rural population
The Trust employs over 5,916 staff and serves a population of approximately 411,000, including residents of Chester, West Cheshire, Ellesmere Port and Neston, and parts of Flintshire.
