We are newly developing community response hubs, bringing together existing services to care and support patients across the Western Cheshire footprint. Our aims are to support people to return home from hospital as soon as they are medically fit and also to support people at home and avoid a hospital admission where appropriate.

Qualifications
  • Excellent communication and organisational skills.
  • Flexible, can‑do approach and a professional, friendly, welcoming manner.
  • Working knowledge of general office procedures and Microsoft Office packages; full training will be provided on CWP systems.
