This is for pre-approved Clinical Students that will be doing Rotations or Approved Clinical Shadows or Clinical Intern.

Students should already be working with a college that is contracted with Memorial Hospital for clinical rotations.

All students in an approved program accepted by Memorial Hospital-Chester, IL will be hired in the system as a Non-Paid Clinical Student or Clinical Intern.

• All information must be completed as you are applying for an internship or clinical student application; including current email, address, mobile number.
• For vaccination records for ability to be in the hospital, you must provide your SSN and DOB.
• You must include all college information, including address and phone number and degree, certification or diploma you are seeking.

Any student that doesn't complete all required fields could be ineligible to begin at Memorial Hospital-Chester, IL for any and all clinical and rotation needs.

Memorial Hospital-Chester, IL has the right to decline any student from starting or completing internship, clinical rotation, shadows, if professionalism, safety, criminal history, arrest for possible criminal intent, HIPAA concern or violation, inactive in program, attendance, parking violations, concern of security, and other areas of concern for patient, employee and hospital safety measures.
