We are seeking a qualified Hospice Home Health Aide (CNA license candidates only) for our Chesapeake office location. This office provides service to the Greater Hampton Roads area.
Job SummaryThe hospice home health aide is a paraprofessional member of the home care team who works under the supervision of a Registered Nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The aide is responsible for observing patients, reporting those observations, and documenting care performed.
Required Qualifications- Willing to travel to multiple patients’ homes per day to provide bath aide visits.
- Must be licensed to practice as a CNA in the state of Virginia.
- Has empathy for the needs of the ill, frail and impaired.
- Is able to communicate information concerning a patient’s condition clearly.
- Displays initiative to work with minimal direct supervision.
- Must be able to read, write and communicate in English.
- Must have reliable transportation and a smartphone.
- Personal care including:
- Baths—bed, tub, shower
- Back rubs
- Oral hygiene
- Changing bed linen
- Assisting the patient with dressing and undressing
- Assisting with grooming—care of hair, including shampoo, shaving, the ordinary care of nails and skin care
- Assisting the patient with toileting activities
- Assisting in feeding the patient, if necessary
- Keeping patient’s living area clean and orderly
- Preparing and serving nutritious meals.
- Assisting the patient in moving from bed to chair, to wheelchair and in walking.
- Reporting changes in the patient’s needs and condition to the supervising nurse.
- Keeping simple records.
- Performs incidental household tasks essential to the patient’s health needs, such as:
- Light housekeeping
- Making and changing beds
- Dusting, vacuuming and mopping the rooms used by patient
- Tidying the kitchen, bedroom and bathroom
- Shopping for patient if no other arrangement is possible
- Personal laundry of patient if no family member available or able
- Performs range‑of‑motion and other simple procedures as an extension of therapy services as ordered (with appropriate completed/demonstrated skills/competency).
- Records patient care activities daily.
- Participates in case conferences.
- Prioritizes work/care tasks to ensure all duties and patient visits are completed when assigned.
- Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct.
- Attends a minimum of eight (12) hours of in‑service annually.
- Participates in meetings as requested by the Clinical Director/Supervisor.
- Performs other duties as assigned.
- Note:
Aides are never allowed to administer medications.
