Job OverviewDaVita is hiring a Social Worker to support patients undergoing dialysis. As part of a team that values work‑life balance, you will help patients understand their rights and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

As a Social Worker, you will provide counseling, advocacy and education to patients and families, conduct psychosocial assessments, develop care plans, coordinate with the multidisciplinary team, and ensure patients receive appropriate resources and support across the continuum of care.
Requirements- State required license to practice social work in a dialysis setting.
- Master's degree in Social Work (MSW) with specialization in clinical practice.
- Licensed Master’s Social Worker or Licensed Clinical Social Worker (required).
- Knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, Outlook and proficiency with DaVita applications within 60 days.
